The Imago Navigating Transition service offers free impartial advice and support to young adults between the ages of 16 and 25 living with a disability in South West Kent.
The Service offers a holistic approach to future planning enabling young adults with a disability to take control of their decisions, understand their limitations and tackle the barriers that prevent them from achieving their desired goals. For some young adults this will be an opportunity to explore and discover their potential options.
Support is offered on 3 levels:
Information, advice and guidance over the phone or email for young people with disabilities and their families/carers.
A regular support group allowing young adults to prepare for their future sharing their concerns, ideas and experiences with their peers. Sessions are designed to address the needs of the participants and may include workshops, training in managing budgets, and preparation for employment and life skills.
Intensive support – 1:1 visits (maximum 12 weeks) development of person centred support plans. The plans incorporate a range of actions, including statutory and community based activities. Young Adults are supported to explore their options and engage with services who can offer them support in meeting their goals. Thus empowering young people to practice choice and control over their futures.
Support may include: benefits application, direct payments, housing, health, building healthy relationships, social activities, transport, pathways to education, volunteering, work experience and paid employment, and assistance in form filling.
