Medway Wellbeing Navigation Service
The Wellbeing Navigator Service gives you access to a trained Community Navigator, whose role is to help you navigate through health, social care, and voluntary sector support services.
The Service offers free and impartial information, advice and guidance, to help you get the assistance you need. We can carry out holistic assessments with individuals, their families, and carers, helping them to find the right opportunities locally.
What is Navigation?
Navigation refers to linking clients and family members with essential health and community services with the aim of improving wellbeing.
Navigation coordinates care and services involving mental, behavioral and physical health care providers; this enables greater holistic and person-centred care.
Navigation assists people to overcome barriers to care, and increases their engagement and active participation in their own care.
Our Community Navigators can help you if you live in Medway, are 18 years or over and are looking for information and guidance, emotional and practical support, or help to access a range of community, health and social care options.

Going through an assessment process.
Medway Maritime Hospital
We have a Navigator based in Medway Maritime Hospital (MMH) offering information, advice, and practical support to help patients get home safely and get the support they need once safely home.
We can also link people with appropriate local services and organisations who can provide ongoing help, such as:
